  • Send us a minimum of 2 grams per sample and ship according to the conditions you require; i.e. refrigerated should be shipped overnight in a cooler with ice-packs

We cannot test your product without the form. Email the form to info@bioformsolutions.com and include it in your shipment.

Give Us a Heads Up Mark the check boxes on the form for the type of microbe you are using, if you have intentionally added yeast, are using spores or postbiotics (dead cells). We can service you faster and provide you with a CofA reflecting your needs if we know this in advance.

CofA Preferred Language Indicate if you prefer standard or scientific numeration, enumeration per capsule, per tablet, per dose, or otherwise. All CofA’s will enumerate live, injured, and dead cells as applicable for AFU testing. Let us know if you have particular wording for Amazon CofAs.

The “Notes” Section Use this section to help ensure solid communication and documentation.

  • Rush? We can guarantee same day service. Write “RUSH” in the notes section of the submission form and we will prioritize your testing and increase staffing as needed. Rush testing has an additional cost.
  • Shelf Life Studies/Stability Testing? We offer both long term and accelerated stability testing. Write in temperature and humidity required and the time frames requested.
  • Yeast Enumeration? We can enumerate yeast if relevant on a separate page of your CofA. Indicate if that is your preference upon submission.
  • Spores? Spores can be dormant or germinated and their state can affect your numbers. Knowing spores are involved in advance can help us with the analysis prior to creating your CofA.
